To enjoy this discount, you must be enrolled in one of our prescription drug plans and complete and return the application to us for processing within the designated timeframe. You will be automatically enrolled in the ExtraCare Health program as long as you remain a member of one of our plans.

The value of the ExtraCare® Health Card:
ExtraCare Health Cardholders receive a 20% discount on regular/non-sale priced CVS Store Brand health-related items at more than 6,900 CVS/pharmacy stores nationwide. To receive your discounts, you just need to present your CVS Caremark† ExtraCare Health card to any CVS crew member at the time of purchase. You may use your card online at www.cvs.com. Enter your ExtraCare Health number (number can be found under the barcode on the back of your card) into your CVS.com profile and your discount will automatically be applied at checkout.

CVS Store Brand health-related items include (but are not limited to) CVS/pharmacy® Ibuprofen (compare to Advil®), CVS/pharmacy® Allergy Relief (compare to Claritin®), CVS/pharmacy® Acid Reducer 150 (compare to Zantac® 150), CVS/pharmacy® Nasal Decongestant (compare to Sudafed®) and many others. There are more than 1,500 eligible health-related CVS Store Brand and Exclusive Brand products.

Your discount will apply to purchases of $1.00 or more. Your savings will be indicated on your CVS store receipt labeled, "HEALTH CARD DSC 20.0."

By using the ExtraCare Health Card as part of your SilverScript Insurance benefit program, you acknowledge your understanding that the discount is restricted to the purchase of CVS Store Brand items for your own medical care. By accepting these cards, you agree to abide by the foregoing restriction. Each time you use your ExtraCare Health Card to obtain discounts on regular/non-sale priced CVS Store Brand health-related items, you reaffirm this agreement.

Using the ExtraCare Health Card also entitles you to all of the benefits of the CVS ExtraCare® program.
