Community Services Network of West Tennessee, Inc. (CSN) has chosen SilverScript Insurance Company to provide your Medicare Part D prescription drug coverage and intends to enroll you into the SilverScript plan in approximately 30 days. CSN is teaming up with SilverScript to bring you the same excellent prescription benefits that you currently have. While SilverScript will be your primary coverage provider and CSN will be your secondary coverage provider, you will see minimal changes in your plan. SilverScript, an affiliate of CVS Caremark, is an approved Medicare Part D Prescription Drug Plan provider. Your SilverScript coverage will be effective on or after 01.01.08 as long as the Centers for Medicare & Medicaid Services (CMS) approves your enrollment.

Group Enrollment Means Easy Enrollment!
To enjoy the benefits of SilverScript you will not need to do anything. SilverScript has teamed up with CSN to automatically enroll you in the SilverScript plans, making the enrollment process convenient and time saving.
Once your enrollment has been confirmed and approved by the CMS, we will send you a SilverScript identification card together with a Welcome Kit for your Medicare Part D enrollment for Medicare Part D Prescription Drug Plan coverage. You can begin taking advantage of the SilverScript drug discount on January 1, 2008.

Secondary Coverage Information
The value of the SilverScript plan is that SilverScript will be able to administer both the Medicare Part D plan and secondary coverage provided by CSN in a relatively seamless fashion. The overall benefit to you is that this will minimize the submission of paper claims, you will continue to have the same $0 co-pay, drug coverage, discounts on certain over-the-counter (OTC) medications and all of the benefits that CSN has brought to you in the past. The only difference is that SilverScript will be providing Medicare Part D coverage coordinated together with the CSN benefits under a single SilverScript ID card.
You may choose to disenroll in SilverScript if you choose, but you will lose valuable secondary coverage provided by CSN. Before you disenroll, CSN requests that you contact Lorna Knight at 1-888-276-9299 or at 1-901-266-7276 to discuss disenrolling.
REMINDER:
- The supplemental coverage is only available with the SilverScript Prescription Drug Plan offered through CSN. You will not get this same benefit if you enroll with another Prescription Drug Plan as your primary coverage.
- Inform SilverScript Insurance and CSN whenever your insurance status changes. This will allow all payers to process your claims correctly and avoid delays in processing.

Retail — Simply present your SilverScript identification card to the pharmacist when filling your prescription at one of 59,000 retail pharmacies in our nationwide pharmacy network.
Mail — Your mail service prescriptions will be filled by a Caremark Mail Service Pharmacy. Caremark has 21 years of experience providing mail pharmacy services with over 1,000 registered pharmacists and pharmacy technicians working to ensure that your prescription fully meets clinical adherence guidelines. Be sure to have your SilverScript identification card handy to include your identification number when placing your order online at www.caremark.com, by phone or mail service form.
